The possible explanations I can think of are: 1) The crystal oscillator from which all timing is derived is off, and the crystal or perhaps even the CPU needs to be replaced; 2) There is a problem in the DAC (perhaps op-amp offset) that is making control of the oscillators nonlinear; 3) There is ripple on the supply which is making control of the oscillators nonlinear; 4) There is a problem in the timer causing it to mis-measure the frequency; 5) You neglected to check the master tuning (the master slider with the LED above it turned on), and it's way off.

My questions are: 1) Is it in tune with itself? That is, if you play it in C# instead of C, does it sound clean and in tune with other instruments, or is the tuning all over the place? 2) Does it drift over time? 3) Have you moved the master tune through its entire range?
